TITLE:                INDEPENDENT LIVING SKILLS CASE MANAGER

REPORTS TO:     Manager of Homeless Youth Programs

JOB SUMMARY:
The ILS Specialist/ Case Manager is responsible for establishing relationships with youth, providing case management, facilitating groups and maintaining a safe environment in the center.  This position will focus on working with 18-21 year old at risk and homeless youth who were in out-of- home placement between the ages of 16 and 21.

Duties and responsibilities include, but are not limited to, the following:

  • Provide Case Management services to youth
  • Manage all tasks related to the front desk at SafeZone, as needed.
  • Provide excellent customer service while greeting and checking youth in the center and answering phones
  • Engage with youth at the center in meaningful and respectful manner assisting them with various resources offered at the center or in the community
  • Establish healthy and appropriate physical and emotional boundaries with youth
  • Support healthy adolescent development of youth.
  • Respond to crisis and de-escalate situations in calm and professional manner
  • Provide individualized services to youth grounded in the 9-Principles guiding to overcome youth homelessness (Trauma-Informed, Holistic, Journey- Oriented, Trusting Youth-Adult Relationships, Positive Youth Development, Strength-Based and Collaboration)
  • Provide basic needs/survival aide, crisis intervention, supportive services, information and referrals to runaway and homeless youth that enable the youth to relocate from the streets to safe and stable housing situations

    • Conduct individual assessments with youth to determine needs and housing stability
    • Assess need and process rental assistance for youth
    • Assist youth in accessing emergency shelter
    • Refer youth to SafeZone and other agencies for supportive services, case management and mental health counseling
  • Complete SafeZone and ILS intakes with youth, including assessments and goal planning
  • Assist clients in meeting benchmarks of their outcomes as outlined in grant and track and report this progress
  • Develop and maintain Independent Living Skills group curriculum and implement Independent Living Skills groups
  • Document youth outcomes in all appropriate databases.
  • Collecting and recording appropriate data and completion of paperwork in a timely manner.
  • Maintain the daily flow of the drop-in center by working in the clothing closet, restocking center supplies, upkeep of the drop-in center, and special projects that arise

    • Assist in the kitchen, prepare meals, and clean-up
  • Comply with agency’s written employment policies
  • Develop and maintain relationships agency-wide and community-wide with youth serving programs and agencies
  • Collaborate with other staff and maintain team work style, and maintain professional working relationships with clients or other outside agencies or contacts
  • Comply with the agency’s written employment policies

TYPICAL PHYSICAL DEMANDS AND WORKING CONDITIONS:

  • Must be able to travel in all weather conditions in MN
  • Able to perform duties and responsibilities as outlined on the job description

JOB QUALIFICATIONS:

  • Minimum of B.A. degree in social services or extensive experience in lieu of degree
  • Minimum of 2 years experience working with the homeless or at risk youth
  • Ability to work sensitively and effectively with adolescent clients from every socio-economic and ethnic group represented in our community
  • Excellent interpersonal and communication skills, including the ability to present information well
  • Valid driver’s license and current automobile insurance required

Hours and Benefits:

This is a full-time position, at 37.5 hours/week. This position requires an availability of evenings and weekends.  Face to Face offers benefits which include health, dental, and life insurance as well as generous PTO and holidays.
