TITLE:       Clinic Provider (part-time)

REPORTS TO:      Clinic Operations Manager

JOB SUMMARY: Face to Face Health and Counseling is a non-profit community clinic providing medical care for young people ages 11-24.  Our patients are primarily from underserved and minority populations typical of East St. Paul.  The Clinic Provider (APRN, WHCNP, FNP, or PNP), in collaboration with clinic staff, provides individualized primary healthcare services by diagnosing and treating certain acute or chronic health conditions typical of adolescents.  The majority of our services are related to reproductive healthcare, immunizations, family planning, pregnancy care, STIs, and minor illness or injury.

Duties and responsibilities include, but are not limited to, the following:

  • Obtains patient history and performs assessment via observation, interview and examination;
  • Orders, performs and interprets diagnostic studies;
  • Performs preventative health assessments, screening, immunizations and patient care;
  • Provides direct treatment and management of health conditions via prescriptive authority or referral to other healthcare providers or community resources;
  • Performs clinical procedures approved by supervising provider or Facility-Based Protocol;
  • Counsels and educates patients and their families/caregivers concerning preventative health, treatment options and community resources;
  • Documents delivery of health care and nursing processes in accordance with specific specialty area standards and network nursing policy;
  • Preserves the patient’s control over decision making, assesses the patient’s commitment to the jointly determined, mutually acceptable plan of care, and fosters patient’s personal responsibility for health;
  • Participates as an integral member of a team by promoting and demonstrating collaborative practice behaviors;
  • Engages in activities that promote professional growth and development to enhance processes that contribute to effective, efficient and safe patient care;
  • Provides patient care in the context of our nine guiding principles for helping youth overcome homelessness (journey oriented, trauma informed, non-judgmental, harm reduction, trusting youth-adult relationships, strengths-based, positive youth development, holistic, collaboration); and
  • Performs other duties as assigned.

    The responsibilities outlined in this job description serve as a general guideline for the position and is not exhaustive of all duties inherent to the Nurse Practitioner position. The licensing Authority Agreement or Facility-Based Protocol further clarifies job duties/responsibilities.

Qualifications

  • Master’s level degree or NP certification as required by your governing board;
  • Current Advanced Practice Registered Nurse (APRN) licensing in Minnesota required or must be obtained prior to hire date;
  • Current Registered Nurse (RN) license to practice in the State of Minnesota is required or must be obtained prior to hire;
  • Ability to work sensitively and effectively with adolescent clients from every socio-economic and ethnic group represented in our community, who may present a broad spectrum of physical and psychological health issues;
  • Two years of specialty experience relevant to the position preferred;
  • Passion for working with at-risk youth population;
  • Proven ability to work sensitively and effectively with diverse populations;
  • Ability to maintain composure in stressful situations;
  • Strong team player and strong people skills; and
  • Positive attitude and willingness to go above and beyond to address patient needs.

    Note: All providers must be credentialed with the MN Department of Health and other insurance carriers Face to Face contracts with prior to starting the position.  The Face to Face billing team will assist with the credentialing process.

    Typical Physical Demands and Working Conditions:
  • Requires standing and walking for extended periods of time;
  • Requires working under stress in emergency situations;
  • Exam rooms and medical offices;
  • Exposure to communicable diseases, toxic substances, medicinal preparations and other conditions common to a clinic environment;
  • Requires manual dexterity and coordination sufficient to operate a copier, calculator, telephone, scales, thermometer, and sphygmomanometer; to administer oral and IM medications;
  • Responsibilities may involve occasional lifting of equipment/supplies, usually less than 25 pounds;
  • Must be able to perform the duties outlined on this job description; and

    Hours and Benefits
    This is a part-time position at 13-14 hours per week (Tuesday 11:30 – 8 pm, Saturdays 10-3pm, and occasional Monday mornings for team meetings).
